From a medical perspective, pruritus ani (persistent itching around the sphincter) is a symptom and not the diagnosis of a condition. It is a common problem which is more prevalent in people over the age of 40 and more common for men than for women. It can, however, affect people of all ages including children. The urge to itch tends to occur more commonly after going to the toilet for a bowel motion and/or at night. Aside from the fact that continual scratching can damage the sensitive skin thereby increasing the likelihood of infection, the problem can interrupt sleep patterns and be a cause of embarrassment. Pruritus ani has many causes and is only seldom due to a serious underlying medical condition. It’s causes can include, but are not limited to: – Sphinctal dysfunction (a small amount of faeces escapes or leaks and causes the itching); – Residual stool deposits after bowel movements; – Skin conditions including eczema, psoriasis and dermatitis (inflammation of the skin); – Excessive moisture in the area due to perspiration; – Reaction to soaps, perfumes, creams, etc; – Infections and diseases; – Fissures, Haemorrhoids & Piles; – Some food types (eg: chillies or other spicy foods, tomatoes, citrus fruits); – Some medicines (eg: chemotherapy); – Threadworms or tapeworms; – Tumours in this area Nearly all of the underlying causes of pruritus ani cause inflammation which then leads to itching. Where a specific underlying cause cannot be identified, this is known as “idiopathic pruritus ani”. “Idiopathic”, in this instance, means unknown or apparently spontaneous origin. In many cases pruritus ani can continue after the underlying cause no longer exists. In these cases, continued scratching causes the body to release inflammatory chemicals which increases redness and itchiness, thereby stimulating the urge to scratch again. This is known as the “itch-scratch” cycle. Similarly, trying to keep the area clean using soaps or using anal ointments which certain ingredients can exacerbate the problem creating a vicious and unending cycle. Is there a cure for Pruritus Ani? There isn’t a single, specific treatment that will cure pruritus ani because it can have many different causes and how long the itching lasts depends on its cause. In many cases it may be impossible to establish its cause. Where the cause of the irritation can be identified it will be possible to treat this appropriately.
